New clients can self-refer directly by filling out the form below.  Our Client Services Coordinator will follow up with you to answer your questions and match you with the therapist best suited for your unique situation.  Third-party referrals from healthcare and related service providers are also welcome.  Referral sources can communicate client information by phone, fax or mail, all of which are listed on this page. 

Due to a high volume of service requests at this time, we will respond to emails and voicemails as quickly as we can, but you may experience a short waiting period.

If you need to reschedule or cancel an appointment, please contact your therapist directly with the contact information they provided to you or by looking on the "About" page. Please note that our cancellation policy requires 24 hours notice. Insurance companies do not cover the cost for late cancellations or missed appointments, therefore you are financially responsible for the session.
